Our service is fee-for service basis Visit Fee:                       New patient $125                       Renewal=    $100 Phone Renewal is available for patients evaluated at one of our offices in a prior year. Will include evaluation by a healthcare professional at one of our clinics. Recommendation will be issued by a licensed MD and valid for a maximum period of one year. We accept payments by Credit card, Money order or Cashier check Only.
